If a tester issues a pass for a testable item which should have failed (minor – 10 points level). How should he or she be disciplined?

Same question… if a pass was issued on a failed vehicle with significant (pootentially dangerous) issues?

Verbal warning?
Formal written warning?
Gross misconduct?
Of course, it depends on the severity of the violation and the tester’s work history, but if it’s a minor offense (10 points or less), it can start with a verbal warning or reminder — along with a retraining session if necessary. But if the vehicle has a serious, safety-threatening defect and is still issued a pass, it can’t be taken lightly. In that case, a formal written warning is the minimum, and if there’s an element of intentionality or repeat offenses, it can be considered a serious offense.
